Colorectal Cancer Prevention Program
Mission
To increase colorectal cancer (CRC) screening rates among persons aged 45 to 75 within partner health systems.
What we do

Offer assistance to the Federally Qualified Health Centers (FQHCs) in the state, in conjunction with the American Cancer Society (ACS), to establish and build on current colonoscopy referral infrastructures with gastroenterology practices and hospitals. Our collective goal is for all age-appropriate patients to have the ability to get screened for colorectal cancer, regardless of insurance status.

Provide resources, training, and guidance to identified colorectal Patient Navigators (PNs) within each FQHC with the goal of helping patients overcome barriers to colorectal cancer screening. Also, closely track patient navigation activities in order to identify best practices and measure the effect on screening rates.

Work with the Data Consultant Contractor to provide all FQHCs with technical assistance on their Electronic Health Records (EHRs) and/or methods for improving clinical workflow.

Implement Evidence-Based Interventions (EBIs) recommended by the Guide to Community Preventive Services (The Community Guide) to increase CRC screening rates in partner health systems. The four priority EBIs are:
- Patient reminders
- Provider reminders
- Provider assessment and feedback on CRC screening rates
- Reducing structural barriers for patients

Improve CRC screening rates using public health strategies from the CDC’s Colorectal Cancer Control Program(CRCCP).
