Immunization Records
How to request a copy of my immunization record
RIDOH does not provide copies of immunization records to members of the public. Immunization records can only be released to healthcare institutions, childcare facilities, schools, colleges, or employers. Members of the public should contact their healthcare provider, childcare facility, school, college or employer to request a copy of your immunization record.
For individuals born after 1997:
RIDOH started collecting immunization information in 1997 using KIDSNET, Rhode Island’s secure database for children’s critical healthcare data. This registry has expanded to include adult immunization information using the Rhode Island Child and Adult Immunization Registry, or RICAIR.
Rhode Island primary care professionals have access to KIDSNET/RICAIR and can provide you with a copy of your immunization record.
Rhode Island school nurse teachers also have access to KIDSNET/RICAIR to check if students have met school vaccination requirements.
Please contact RIDOH.RICAIR@health.ri.gov for assistance sending records to out-of-state healthcare institutions or school/college health services.

For individuals born before 1997:
RICAIR has very limited immunization information from the years prior to 1997.
The best source for vaccination records is your primary care office. However, they may not have records from your childhood.
Primary care healthcare professionals have access to RICAIR and may have additional information from other sources. Many primary care offices have patient portals where you can access your immunization record.
COVID-19 vaccination records can be accessed at portal.ri.gov/VaccineRecord or by downloading Rhode Island’s public health mobile application 401Health.
Since the Fall of 2020, pharmacies and some healthcare institutions began sending information about vaccines recently administered to adults in Rhode Island to RICAIR. Rhode Island primary care healthcare professionals have access to KIDSNET/RICAIR and can provide you with a copy of your vaccination record.

The Rhode Island Medical Society may have information about where records from a deceased or retired physician are stored. Gorwood Medical Systems and Iron Mountain are the most common record storage locations.
